Report-A-Problem (RAP) App on a PC or Mac

 

 

Please read the instructions below on how to use the RAP App on a PC or Mac.

 

For instructions on how to use the app on a mobile device such as a smartphone or tablet, click here

 

1. Click here to launch the app. The app will open in a new browser tab.

 

2. Enter your UTHSCSA Email Address and then click on the Next button -

 

 

 

3. The app will then appear. The Name, Phone and Email fields are automatically filled in with your information. Use the vertical scroll bar to scroll down the app to make your entries.

 

 

 

4. Select the Type of Problem. Choices are Temperature, Electrical, Plumbing, Building, Custodial, Access Control, Grounds & Other -

 

 

 

5. After making a selection, provide details of the problem you want to report -

 

 

 

6. Using the Location dropdown menus, specify the location of the problem. Providing a complete location helps us plan our work faster and assign and dispatch technicians to the proper site.

 

 

 

7. If you are unable to specify a location using the menus above, you can use the 'Additional Location' box to enter comments and give us the location -

 

 

 

8. Click on the Submit button to complete filling out the app -

 

 

 

9. You will be asked to confirm your submission or you can make changes to your entries. Click Confirm.

 

 

 

10. Click on the Exit button to complete the submission -
