Remodeling
Select REMODELING from the Types of Work page then click on the NEXT button
The initial section of the Remodeling form will prompt the requestor with choices. Each selection is discussed below.

Select this option if Facilities Management has previously provided you a Work Order. Enter the Work Order number in the indicated boxes then click on the NEXT button.
A section of the form will appear discussing the guidelines and requirements that need to be completed. Click inside the 'OK' checkbox to acknowledge your understanding of the requirements.
Prior to submitting the request, you can provide details, instructions, and other information in the space indicated.
'I Need An Estimate' and 'I Do Not Need An Estimate'
Clicking on the checkbox for 'I Need An Estimate' will present the requestor with the 'Explanation Of Estimates' information section of the form (blue box). Click inside the 'OK' checkbox to acknowledge your understanding of the explanation.
NOTE: This is the only difference between the 'I Need An Estimate' and 'I Do Not Need An Estimate' options.
Room Finishes
The 'Room Finishes' section of the form will appear. This comprehensive list will aid the requestor in specifying which items that will be required for the Remodeling request. The chosen finishes will also aid Facilities Management in formulating an estimate. By default, all Room Finishes are in the 'No' position. To explore the options for each Room Finish, click inside the 'Yes' button.
For example, if you click on the Yes button for 'Wall Paint Required' as shown below, you will be presented with options to select a Base Paint color and an Accent Paint color. Use the list menus (blue arrow) to select a paint color. A swatch of the color will appear to the right of the list menu.
In some of the Room Finish selections, you will be asked to supply information. For the 'Electrical Required' selection shown below, you are expected to enter quantities for different types of electrical outlets.
For the 'Utilities Required' selection, various types of utilities services and systems can be selected. Click inside the checkbox of each that is required..
Other Room Finish selections, such as those for 'Security Cameras' and 'Audio Visual Equipment' as shown below, are for informational purposes only. They are included in the Remodeling form to advise the requestor that the service is not provided by Facilities Management. The requestor will need to acquire these specific services from the proper departments.
The 'Additional Comments' section allows you to provide details, instructions, and other information in the space indicated.
Print Screen page
After clicking on the NEXT button, the Print Screen page of the Service Request form will appear. This page will show the completed Department Information and Remodeling sections of the form and will allow the requestor to print the form if desired. A blue banner on top gives printing instructions. This is the only time that the form can be printed.
Submitting the Service Request
Click on the SUBMIT button. A small page will appear confirming the action. This page informs the requestor that email notifications will be sent based on events that will occur as the service request goes through its workflow. Click on the CLOSE button to close the page.
Service Request Workflow and Approval Process
As part of the Service Request workflow, emails will be sent to the requestor and approver.
The email sent to the requestor will confirm that the request has been successfully submitted. It will contain the Service Request number and a general description of the work requested.
The email sent to the approver will contain the Service Request number, a general description of the work requested, and the name of the requestor who submitted the request. The email will also contain a link to the Service Request form. When the form is opened on the approver's internet browser, the approve will decide whether to approve or disapprove the request.
